Job Title:  Family Assessment Worker
 
Department:  Healthy Start Program

Summary
:  Responsible for the screening, assessment and referral of prenatal women and families with infants under the age of 1 year to the Healthy Start Program. Also prenatal women and families with children under the age of 2.5 years who have been accepted by the Department of Human Services Child Welfare Services or Voluntary Case Management of Family Strengthening Services. The Family Assessment Worker is an important part of an organized membership that strives to maintain its' professional leadership at all levels of administration professional leadership.

Essential Duties and Responsibilities include the following. Other duties may be assigned.
Screens all births at the Hilo Medical Center. Determines whether the details provided in the screening necessitate a face-to-face psychosocial needs assessment. NOTE: Weekend work is required to fulfill this job duty.
Conducts monthly up to 25 psychosocial needs assessments with families within the eligibility period and make determination as to eligibility status.
Plans appropriate service intervention disposition for families and submits information to the Family Assessment Clinical Supervisor for review.
Make appropriate referrals to needed community resources.
Initiates participant record for documentation and monitoring.
Provides bonding facilitation assistance as appropriate.
Serves as a participant advocate as necessary.
Plans and provides short-term direct assistance to family as appropriate.
May assist in training of Hilo Medical Center staff on the YWCA Healthy Start Program.
Meet regular with the Family Assessment Clinical Supervisor to discuss procedures, policies, intervention and participant needs.
Participant in and contribute to Family Assessment team meetings.
May be asked to attend hospital case conference meetings regarding service delivery to participants.
Maintain participant confidentiality.
Demonstrates skills in building rapport with patients and a comprehensive description of the Healthy Start Services and the YWCA of Hawai'i Island Healthy Start Program.
Prepares regular statistical and narrative reports for the YWCA of Hawai'i Island Healthy Start Program, Maternal Child Health Branch (MCHB) and Felix Decree.
Refer appropriate cases to the appropriate Home Visiting Clinical Supervisor within 48 hours.
Demonstrate the ability to establish a good working relationship with team members, other co-workers, supervisors and staff of the YWCA of Hawai'i Island.
Sign in and out on the board in the front office when entering or exiting the office.
Maintain work schedule as arranged by the Family Assessment Clinical Supervisor. Any changes must be pre-approved by the Family Assessment Clinical Supervisor.
Observes the Hilo Medical Center's dress code, wears identification badge and maintains a professional appearance at all times.
Prepare regular statistical narrative reports.
Attends all staff meetings and trainings.
Conducts weekly contact with staff from the following agencies; Women Infant Children (WIC), Malama Perinatal, Pediatricians, OB-GYNs, Clinics, Public Health Nurses (PHN), and other referral sources.
Coordinates and participates in East Hawaii community fairs and events as assigned by the Clinical Supervisor.

Education and/or Experience:  High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Other Skills and Abilities:  Organized with attention to detail, flexible, and self-directed.
Possess excellent communication skills, ability to train staff, and be a team leader.
Demonstrate understanding of child abuse and neglect dynamics, child development and parent-child interaction.
Must have knowledge of the community and related issues and concerns; culture and customs; organizations, groups, and associations interested in and/or servicing our community in East Hawai'i.

Other Qualifications:  Must have a valid Hawai'i Driver's License, dependable automobile and current auto insurance with Bodily Injury liability minimum of 100/300. Family Support Worker – Healthy Start Program
Summary Aids individuals and families having problems concerning family relationships or other aspects of their social functioning affecting unity of family and welfare of community by performing the following duties.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Contacts families to schedule a visit with the family.

Consult with the professional members of the team to develop approaches to address the family’s risk factors.

Foster a supportive relationship with the family.

Provide services to meet the needs of culturally diverse families.

Participate in staff meetings and child team meetings to provide current information on the family’s status to decrease their risk factors.

Completes service documentation in accordance with the standards and timelines established using systems and tools provided by the YWCA.

Communicate on a regular basis with the professional team members of family’s needs/concerns.

Communicate with the professional team members regarding significant family observations, e.g. safety issues regarding the child, mental health and/domestic violence issues, or substance abuse issues.

Participate and support families in the Child Development Evaluation (CDE) process with guidance from the Clinical Specialist and Child Development Specialist.

Support families transitioning to other programs.

Update the Family Support Plan (FSP) review for children not receiving early intervention therapeutic services.

Attend and participate in all FSP meetings.

Refer families to community agencies, such as Department of Health Services (DHS) or Community Clearinghouse.

Implement strategies in the FSP with guidance from the professional members on the team.

Work with the CDS to conduct approved development screens and Nursing Child Assessment Teaching Scale (NCAST) scales.

Provides home visits and support services to Healthy Start participants according to policies and procedures of the program.

Completes contract, programmatic, and organizational paperwork according to the policies and procedures of Healthy Start Program and the YWCA of Hawaii Island.

Attends all required staff meetings and trainings.

Performs other related duties as assigned.

Education and/or Experience:  Bachelor’s degree in social work or human services, preferred. High school diploma or general education degree (GED).  Two years direct client service (home-based preferred) or child rearing experience and/or equivalent combination of education and experience.

Preschool Teacher for our Developmental Preschool
Location is downtown Hilo.
Summary:  The Preschool Teacher is responsible for the coordination, planning and implementation of individual and group developmentally appropriate activities to stimulate growth in language, social, emotional and motor skills in young children by performing the following duties in a membership organization.

Essential Duties and Responsibilities include and are not limited to the following. Other duties may be assigned.

A. Teaching

Plans and prepares curriculum and daily activities for children in conjunction with the Lead Teacher.

Works with children implementing planned curriculum and daily activities.
1.  Able to plan and lead large group meetings, activities, transitions etc.
2.  Plans, prepares, implements and evaluates small group and interest activities.
3.  Plans, prepares, implements and evaluates program curriculum through interest center.
4.  Prepares and presents group storytelling.
5.  Plans and conducts outdoor games or activities once a week.

4.  Maintains continuous visual and auditory supervision of children at all times to insure their safety in the classroom, on the playground, pool and field trips.

Staff Development
Participates in staff meetings and planning times.
Provides input on selection of program supplies and equipment.
Assumes assigned administrative responsibilities in absence of the Lead Teacher, as assigned.
Participates in quarterly staff reviews and goal setting process.
Maintains professional development by participating in training provided by agency and other organizations as available.
Assist in conducting in service training and/or parent workshops as needed.
Provides on-going feed back to supervisor as to job performance and program evaluation.
Meets and continues to maintain all Department of Human Services and program position requirements.

Other Functions:
Assist with fundraising.
May be asked to represent YWCA organization and preschool at various community groups or functions. May be assigned to carry staff responsibility for special events or their projects within the YWCA. Assist with night work when necessary.

Must be certified in infant/child first aid including rescue breathing and first aid for choking. Must have physician's clearance to perform duties. Job Title:                  Clinical Specialist
Department:             Healthy Start Program

Summary Coordinates and provides technical expertise to staff in a specialized patient care program by performing the following duties.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Upon referral from Clinical Supervisors, conducts home visits for each family and
collects information about clients (individuals, married couples, or families), using interview, case history, and observation techniques, and appraisal and assessment methods.

Analyzes information collected to determine advisability of counseling or referral to other specialist or agencies.

Conducts professional development sessions for all YWCA Healthy Start Program staff on family and child rearing dynamics.

In conjunction with the Clinical Supervisor, provides clinical consultation during case conferences with the Family Support Worker.

Consults reference material such as textbooks, manuals, and journals, to identify symptoms, make diagnoses, develop therapeutic or treatment plan.

Provides counseling sessions to families to help them identify and implement solutions to their concerns.

Counsels’ participant’s home with counseling methods and procedures, such as brief therapy or solution focused therapy, to assist participants in gaining insight into personal and interactive problems, to define goals, and to plan action reflecting abilities, needs, and goals of service plan.

Evaluates results of counseling methods to determine reliability and validity of treatment used.

Conducts a Concerns Inventory with the referred family in order to identify family stressors and to form the basis for support.

Conducts a Concerns Inventory with the family upon conclusion of counseling sessions to ascertain the impact of services on the family.

Participates as part of the Clinical Supervision Team in developing and refining program policies and procedures pertaining to family intervention.

Conducts parenting and support groups for program families.

Participates in case conferences with other community service agencies.

Interacts with other professionals to discuss therapy or treatment, new resources or techniques, and to share information.

Submits regular written reports and summarizes to Clinical Supervisors regarding observations, interventions, progress and activities of referred participants.

Provides regular narrative and statistical activity reports to the Clinical Manager.

Must have current knowledge and working experience in child abuse and neglect programming.

Job Title:            Home Visiting Clinical Supervisor
Department:       Healthy Start Program

Summary:  The Home Visiting Clinical Supervisor under the general direction of the Program Director, the Home Visiting Clinical Supervisor is responsible for the direct supervision of daily activities of the Family Support Worker team including administrative and service delivery components at the YWCA of Hawai'i Islands by performing the following duties.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Coordinates with and assists the Program Director in providing the initial and ongoing  in-service training for Home Visiting team.

Provides clinical supervision of Family Support Workers including case reviews, case planning, and case management as appropriate.

Reviews and approves all case documentation on an on-going basis and assures that any cause which may involve "imminent harm", "threatened Harm", abuse or neglect is referred to Child Welfare Services (CWS).

Ensures close coordination between Home Visiting staff and all agencies and individuals involved with assessment of participants to assure maximum utilization of services by participant.

Provides information regarding program activities and policies to other organizations and the general public.

Attends case conferences as necessary.

Maintains participant confidentiality and oversees Home Visiting staff in assuring confidentiality.

Meets regularly with and demonstrates on-going availability to assigned Home Visiting staff to provide supportive supervision, guidance and monitoring of progress.

Assures all contract obligations are met including defined service activities, target group numbers and designated participant assessments.

Maintains required data collection systems for program, including information for program monitoring reports and program evaluation.

Analyzes data on ongoing basis to assist in program planning.

Participates in and contributes to team meetings.

Participates in program management team meetings.

Facilitates in community ad state planning in regard to child abuse treatment and Healthy Start service delivery.

Develops policies, procedures and protocols as needed to carry out activities. Prepares regular statistical and narrative reports.

Attends all required staff meetings and trainings.

Supervisory Responsibilities:  Directly supervises 5 to 6 employees/Para-Professional and Professional. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Rate of Pay:  $36,000 to $38,000 yearly, exempt position.
